Volunteer Health Clearance Forms

Annual Tuberculosis Screening

Purpose: Volunteers are required to take a Tuberculosis test each year during the month of the volunteer’s birthday. This form should be printed out and taken to the Occupational Health Office (Main building Rm MG 418) as documentation of the test.

Instructions: The Occupational Health Office does not require appointments for annual TB tests. After the TB test is placed, the volunteer must return to Occupational Health in 48 hours to have the results read and documented using this form. Once completed, a copy of this form
should be submitted to the Volunteer Services Office via email or the drop box in C3402.

Adult: Adult volunteers are required to have two TB tests prior to service. Occupational Health can provide the second TB test at no cost to the volunteer. This form should be used to document the results of the second TB test as well as annual testing.

High School: Annual TB tests are required. Results should be documented using this form.

Health Screening Form

Purpose: This form should be completed prior to service at MedStar Georgetown and does not need to be updated annually.

Instructions: Volunteers should submit this form to their primary care physician or other health care provider. Once completed, volunteers should submit a copy directly to the Volunteer Services Office via email or the drop box in C3402. This form is due within two weeks of acceptance into the volunteer program.

Adult and High School Volunteers: A Health Screening form is required prior to service.

Volunteer Agreements

Background Check Authorization

Purpose: It is a requirement of MedStar Georgetown that all associates and volunteers undergo a background check as a measure to screen individuals who will have access to patients and patient information. Prior to initiating a background check, volunteers must provide consent.

Instructions: The online application will give volunteers an opportunity to provide consent for a background check by checking an “I agree” check box. Volunteers should read this form in its entirety to understand all terms included in this authorization.

Adult: Only Adult volunteers (18+ years) are required to pass a background check prior to volunteer service and must check the “I agree” check box in the online application.

High School: All volunteers under 18 are not required to undergo a background check and should have checked the "I am under 18" check box in the online application.

MedStar Georgetown Property Agreement

Purpose: MedStar Georgetown provides identification badges and volunteer jackets for all volunteers to utilize while serving at the hospital. The purpose of this agreement is to ensure that these items are returned upon completion of service. This enables MedStar Georgetown to accurately track and distribute resources to volunteers currently serving.

Instructions: The online application will give volunteers an opportunity to accept these terms by checking an “I agree” check box. Volunteers should read this property agreement in its entirety to understand all terms included.

Confidentiality Statement

Purpose: MedStar Georgetown volunteers and associates are trusted figures in the hospital and community and have access to confidential and sensitive information. This statement ensures you understand and accept your responsibility and obligation to keep this information confidential.

Instructions: The entire contents of this Confidentiality Statement are described in the online application. All volunteers must check the “I agree” check box to accept the role of protecting patient privacy. All volunteers should also read through Policy #456 listed in the Orientation Materials section.

Adult and High School Volunteers: Agreement to these terms by clicking the “I agree” check box in the online application is required prior to service.

Commitment & Personal Statement

Purpose: Serving as a MedStar Georgetown volunteer is a serious commitment. By signing this form, the volunteer accepts the responsibility and duties of his/her assignment, schedule and commitment of hours.

Instructions: The online application will give volunteers an opportunity to accept these terms by checking an “I agree” check box. Volunteers should read this statement in its entirety prior to committing to volunteer at MedStar Georgetown.

Policy #456: Confidential Patient Information and Patient Privacy

Purpose: This MedStar Georgetown policy outlines the procedures of governing patient information according to legal, accrediting and regulatory agencies.

Instructions: Volunteers should read through this policy as a part of the MedStar Georgetown orientation.
