Preparing for Your Appointment
A prescription for pediatric evaluations and therapy is required. A specific diagnosis for why your child is receiving this evaluation should be included on the prescription (e.g., hypotonia, language delay, brachial plexus injury). We must receive this prescription prior to scheduling your appointment. It is the parent’s responsibility to obtain a prescription from their child’s physician prior to contacting our office to schedule an appointment. Prescriptions must be dated within 90 days of your child’s appointment. Prescriptions can be faxed to 1-855-470-6848.
Please be aware that new patient appointments are scheduled two to four months in advance. Also, be aware that once your child’s evaluation is complete, your therapist will indicate if therapy is recommended. At that time, your child may be placed on a waitlist for regular therapy services.
After you have scheduled your child’s appointment, please call 202-295-0546 if you need to cancel or reschedule his/her appointment. An appointment should be cancelled if your child is ill. Our department will try to reschedule the evaluation within a reasonable time frame.
All evaluations and therapy are conducted in the Physical Medicine & Rehabilitation Department.
